2. 3(Three) colour photographs of the child alongwith both the parents.
3. 3(Three) passport-size colour photographs of the child.
4. Photocopy of original mark sheet of the last examination passed.
5. Original Birth Certificate.
6. Original Transfer Certificate signed by the competent authority.
